As well as the upper airway, CPAP pressurizes the inner ear and all of the sinuses. This introduction of pressurized air is bound to increase the probability of infection. I would also imagine that some people would be more prone to such infections than others. I suppose it's also possible that your inner ear is just very sensitive to the the air pressure, or maybe there is some particular anatomical reason that makes your inner air react to CPAP air pressure.
You don't say whether you are using a heated inline humidifier, but use of such an appliance very frequently eliminates infections, discomfort, pain and bleeding from dried sinues that is an almost invariable result from using CPAP without a heated inline humidifier. Essentially, a heated humidifier should always be prescibed with CPAP.
