One possible cause (and it is only a layman's guess, of course) is nasal polyps. I went to several regular doctors and an ENT who all prescribed antihistimines and decongestants -- which didn't help. Finally my sleep doctor suggested that it was something else "behind" my nasal passages, and sent me to an ENT who specialized in sinus surgery. He actually diagnosed me as he walked into the examination room. A subsequent look in my nose and a CT scan revealed severe nasal polyps. I had them removed and now can breathe better than ever in my life.

I realize that this may or may not be your problem, but I mention it because so many docs missed it entirely. Only the CT scan revealed the problem.
