If you see zero difference after 4 months I would definately recommend another titration study. If you are seeing some difference, just not much and it is coming along slowly, you might simply have a larger than average sleep debt to pay back and it is taking longer. You might also not be getting a enough sleep. Some folks need less than 8 hours, some more. Try getting bed a little early and see if that helps.

Another possibility is your are allowing air to escape from your mouth reducing the effectiveness of CPAP. You can try a chin strap or full face mask if this is a possibility.
