Smoking is especially bad for people with sleep apnea. Sleep apnea already puts stress on your heart; smoking adds more. I would quit as soon as I was able.
What is your sleep doctor's primary specialty? Some of them are ENTs, some pulmonologists, some neurologists, and some psychiatrists. Any responsible sleep specialist should at least look at your throat to see if there are obvious structural issues contributing to or causing your apnea.
I gather that you don't have a diagnosis yet, but hang in there until you do. You need to find out what's going on. In the meantime, educate yourself about sleep apnea, either by reading this forum or by getting hold of "Phantom of the Night," a good, basic book for the non-physician on sleep apnea. You won't have all the problems people talk about, by any means, but it'll help to see what choices people have made and why.
Most of us do a good deal better once we've gotten CPAP and gotten it working for us.
