I have Apria as my DME and find them to be very bureaucratic and disorganized. About 5 months ago I had my sleep doc fax a prescription for a mask to Apria and the rep tried to get me to pay for it and collect from my Insurance. I protested and got my Insurance company to provide a reference # and Apria complied. Recently I got a denial from insurance and checked with DME to find that they had not used the reference number in billing insurance. I suppose they will get the money but I think they charge the higher prices to cover administrative bloopers.
I was told by the rep that often insurance refuses to pay but this exprience would indicate a documentation failure. Anyway, the headgear they gave me with the mask was defective but we made it functional and the rep promised to send me a replacement which I have never received.
