In my case, it's very positional. Definite obstruction problems at the level of the tongue. Titrated at 7cm on my side and a 11 on my back. The ceph (head x-ray) showed a relatively large tongue within a relatively small oral-pharynx cavity with the tongue set in a somewhat posterior position. (They'll take a ceph right in their office and have it developed in 5 minutes. You can also have an endoscopy exam done in addition to this, but beware...it's quite expensive because the folks at Stanford don't participate in any medical insurance plans for some reason....It's all out-of-network!).
In any case, because of these findings, and the fact that my prior surgeries to the palate and the nose had no effect whatsoever, Powell recommended a genioglossus advancement and, if necessary, additional treatments of somnoplasty to the base of the tongue.
It would be interesting to hear what they'll recommend in your case. Your symptoms seem somewhat different than mine...
