Pat - Your ideal mask sounds a lot like the Respironics Profile Lite. It swivels over the head, and the "Whisper Swivel" diffuses the exhaust air over your forehead. Headgear fits under and above the ears. The gel cushion can be custom fitted to your face by putting it in boiling hot water for a few seconds, cooling it, and then fitting to your face.
If you can do without the gel cushion, the Respironics Contour Deluxe is a good bet if you order a Whisper Swivel at the same time. You have to remove the hose swivel on the Contour (just a press fit), stick in the Whisper Swivel and plug the exhaust hole on the mask swivel with a blob from a hot glue gun. You in business.
