For me, the strap that goes over the top of the head was WAY too long--if I shortened it using the velcro, the two upper side straps came right over my eyes.
First I adjusted the velcro so that the two slits in the top strap came at eyebrow height. Then I pinched the top strap with my fingers (gathered up the slack) to see how much length I needed to "remove." Then I put a bulldog clip on the slack to hold it that way. Next I threaded the two upper side straps through the slits (each folds back onto itself) and adjusted them.
It took a little trial and error to find how much slack I wanted to get rid of in the top strap--adjusting with the bulldog made that very easy. When I was content that I had the length right, I market the spot where the clip was, then folded in the excess length and stitched through it using a darning needle and heavy thread (I didn't want to risk doing this with my sewing maching--might end up tearing the strap).
So, top strap over the head, top two side straps at eyebrow-level, and the bottom two straps under the ears.
I use a tiny piece of moleskin on my nose to protect it; moleskin peels off easily and you can reuse it about 4 times. Get moleskin in the footcare section of the drugstore (a couple of small sheets of the stuff for a couple of bucks.
I've had very good luck with this mask and especially like the small "footprint" on my face. My forehead slants back too much (or my nose is too big and long) for the Contour Deluxe, my nostrils got irritated from nasal pillows (despite neosporin), and the Mirage works just fine, but I hate that wet-nose feeling and jet-pilot feel.
As for the port being in the wrong place--I used to suspend the hose over the top of my head, but found this often resulted in the air exhaust aiming right at my chin. To manage this, I used that white velcro tube holder that comes with the Mirage--but this often resulted in too much torque on the nose (which became sore and red). Now I just haul the hose into bed with me and aim the exhaust up over my head. Really works great.
