I have BC/BS and they have a contract with the DME and the DMEhas to accept the contracted price. They can't come back after me. An example, they charged 180 for the heated humidifier, insurance paid 86 and that's all they get. If your DME has a contract with your insurance company they have to accept that. They can ask for higher prices next time they negotiate the contract or decide not to deal with that insurance company anymore. The copays are set too. I pay 20 percent of the allowed amount, not the amount the DME charges, until I have paid 350, and then insurance pays it all.
