That is the impression I've had as well. It seems that it is not definite that it could be the angle of spraying, but there is a high suspicion that it is the manner of use/spraying that is the cause of the perforations.
My understanding is that steroid sprays should be directed into the airways and not toward the tissues lining them.
I have been using steroid sprays for years with good effectiveness and no septal defects so far.
