I have the same set-up that you do. When starting I found that use of the angle adapter caused leaks. After one year, I started having leaks, and added the angle adapter back in and the leaks went away. Go figure!
It seems most people use the large pillows, and that may be another area to check. I have the mediums, large, and dilator pillows, that I have tried. I can't use the mediums.
As to the nose getting bigger, have you seen the cartoon character on CPAPMAN's site. Look at the size of his honker. It does make you wonder, doesn't it? I have never heard from anyone that had to quit using pillows cause their nose got to big!
