No need for any concern: When lying down the hose on the Breeze headgear is free to 'float.' It is captured only by two clips, the last being directly centered over the padded bridge that rests on the curve of forehead to head. The hose has never twisted under my head or been in my way, whether sleeping on my back or on either side. There's an inline swivel that connects the free end of the Breeze hose to the longer / heavier hose from the humidifier.
Bear in mind that I'm biased - I chucked away my nasal mask within 3 weeks. You would have to kill to get my Breeze+pillows from me! If your husband has an engineer's eye, he'll go straight for the Breeze. Straps are medieval.
