How to handle night time on call

I'm moving in to a new position at work and am loving it. One of my responsibilities will involve being backup for systems that run 24/7. My concern about being called in the middle of the night is not so much being able to handle the work, but being a zombie for the next day or so. My apnea is not under control yet. Like last night I woke up at least once per hour. I'm scheduled for another study, but in the meantime..
Is anyone else on call, and what did you tell your employer about needing to sleep in or leave early if you do get called in the middle of the night. I don't mind staying late to complete work if necessary, but disrupting my sleep is a concern to me.
