Sounds like you have the same type of job I do. One thing I noticed with my first middle of the night call on CPAP was that it took me a bit longer to become alert than before CPAP. I think it probably had to do with getting into a deeper sleep than before and having to struggle my way to awake. Before CPAP, I would handle the middle of the night call, shower and go in for a full day and have no problem (other than my memory and concentration abilities). After CPAP, I go back to bed after the call and sleep until I wake up before getting ready and going in to work. My job does not require a full days work in addition to night calls, so I am lucky in that respect. They were aware of my struggles to get treatment prior to Dx and were very understanding. It helped, I am sure, that my boss' husband works with the handicapped. She had an understanding of health difficulties in relation to work.
My advice is to make sure they are aware of your problem and that you are attempting to get the help you need. Find out if you can be flexible in when you put in your 8 hours - most on call jobs allow for you to work shorter hours the day after a middle of the night call. Maybe all it will take is a phonemail or email to your boss when your call is handled stating that you may be sleeping in and not to expect you at your regular time.
