Mirage FF Mask headgear question--help!

I just got my Mirage FF mask in the mail and I can't figure out the headgear. Can anyone clue me in? Which straps go in the sides and which go on the bottom? Right now I have the narrower straps in the bottom slits. Also, there is a cord with lots of little velcro pieces on it that I have no idea what it's for. Help!
