The 15 inch hose should have a swivel adaptor at both ends. The lower swivel fits into the angle adaptor, which then fits into the shell. The upper swivel connects the short 15 inch hose with the long hose.

Sounds like you are missing either the swivel adaptor or the 15-inch hose or both. My guess is that they have connected the long hose directly to the nasal shell.

Why don't you put the new ADAM circuit and the old one side by side and see what is different?
