The easiest way is to pour the vinegar/water or soap/water into the hose and connect the ends together with a hose connector (a short piece of pvc pumbing pipe works fine if you don't have a "real" connector already). Then just hang the filled hose loop over the shower head or even a door knob.

To rinse the soap out, I just hold one end of the hose against the shower head and run the shower through it.

Soaking the entire hose for hours is not really required, by the way. Mine usually hang on the faucet for 15 minutes or so. Then I get in the shower and rinse it while I am showering.

How often to do it? I do it when I remember to do it.
