Sleep apnea can vary from day to day, and it is conceivable that your set pressure is insufficient on days when you apnea is worse. This could explain your intermittant problems.
It could also be caused by mouth breathing or leaks as Fog has mentioned.
The most common cause of this type of problem as this time period of use is related to sleep debt. Sleep apnea causes a disruption of our sleep that leads to sleep depravation, and an accumulated sleep debt. We have to pay that off in order to achieve a full recovery.
