I suggest that you put much effort and care into deciding where to go for the sleep study. Your sleep apnea is of such long duration and apparent severity (from your description) that you should be sure to go to a sleep specialist at a major medical facility. If you can do it, head for a sleep lab that is managed by a neurology department. Pulmonary and ENT specialists tend - in my opinion - to make findings that favor the use of their own specialization skills, whereas neurologists seem to work on a broader landscape and to be readier to work with cardiologists. If you can avoid it, don't go to a service lab that is running patients through their sleep mill on behalf of general practitioners who often know too little about sleep disorders and their long-term consequences.
Read extensively, using the links on this site, and get your act together without delay. Your description of your present status suggests to me that you should very seriously consider voluntarily giving up driving. If you cause a major accident with high $$$ consequences for your insurer, it may repudiate responsibility if its assessor finds out that you have left your apnea untreated for three years. The financial consequences for your family could be devastating.
So - get rolling without further debate. Alleviating your sleep problem is now your most important project, because of its importance to your quality of life and financial security.
You will be making an important lifestyle change. Like giving up alcohol or tobacco, you will succeed more easily if you approach the task knowing that it is easier if you want the benefit, rather than doing it merely because you think that maybe you ought to, or because you are being nagged.
