A prescription from your doctor for a humidifier should do it for having insurance pay for the humidifier. If the insurance balks, put up a fight since the doctor prescribed it.

My DME got verification from the insurance before delivering my equipment. The CPAP is rented rather than purchased. They bill the insurance and I get a bill for the co-pay. When I got the prescription for the humidifier, I was told that it is a purchase item and that I would have to pay whatever the insurance didn't. Haven't gotten a bill for it yet after at least 5 months. Not all DMEs or Insurance companies will work this way, but your DME should probably take care of submitting the proper paperwork, etc.

There is a lot of discussion on different masks, if you do a search on the different forums for masks, or the specific style of mask you are interested in. Same for machines.
