Medicare is administered by different companies based on the region you live in. CMS (the Center for Medicare and Medicaid) formerly HCFA has the NCD change listed as of 10/31/01. I would advise contacting the Medicare office in your region and refer them to #CAG-00093R.[zero's not "O's"]. You can direct them to the following URL
