The best image I've run across for how CPAP works is that it provides "an air splint" in the entire column of the soft tissue of your throat to prevent the closing that creates OSA. It's the gold standard in medical treatment for the problem. One of the reasons surgery is so uncertain is that it's difficult to determine exactly where in that column of soft tissue the problem lies.
