I know of what you speak. Getting used to PAP and getting the thing to work is not easy for some of us. It took me many months - and I still have bad nights where I am left mentally dead the next day.

There are two common problems for this. First - most people have varying pressure needs from night to night. If you need perhaps 12, and your machine is set for 10 - you will not get the treatment you need. Sleep studies are a one time snapshot of your needs. Typically they are fairly good as most people are very sleep deprived and conk out really good once put on PAP (if even for a short time). Also, it is very common for pressure to chang with age. Your 4 year old sleep study could be way off from your current needs.

However, many people have problems even when the pressure is right. This relates to mask leaks, wich includes mouth breathing. PAP therapy is not effective in the presence of major mask leakage. Thus you would still end up feeling dead (this is the problem I occasionaly have even to this day).

The best solution to both problems is one of the better AutoCPAPs with software. They report on mask leakage (so you know when you have a problem or not - and can fix it), and they also can tell you when your pressure needs routinely exceed your titration setting. For this you need the machine maximum set at or near the titration pressure (do not exceed the perscription pressure without permission of your Dr), and the machine will max out at the set maximim more than occasionally for a little bit (we all have occasional bad nights). If this happens take the information back to your Dr and discuss a perscription change.
