Thanks everyone -- I actually got excellent answers from a couple of the vendors regarding the fit. Apparently the important dimension is from the bridge of the nose right between the eyes down to the "dimple" below your lower lip and above your chin. Who knew :)

I think I ended up paying somewhere just below $200. Now the fun begins when I try to convince insurance to pay for it :) And thanks for the Beltsville company -- I'm right around the corner.
