Yes, my insurance "should" pay the for mask if I bought it from a vendor on the sleep mall BUT I would have to submit the forms myself to the insurance as an "out of network" claim. For me, with an "in network" DME, I have to pay a 10% copay in addition to meeting my deductible requirements first. For "out of network" it 20%. Now that sounds cheaper to stay in-network but that all depends on what the in-network DME charges for that mask. Sometimes it works out cheaper to go out-of-network and pay the 20% then pay 10% of the DME's price for that item. When I got my first mirage FF mask(series1) I had a copay of $57. They either didn't charge me for the fitting or that was included with the price of the mask. My series2 I got for free at a drawing during one of the AwakeInPhily meetings I attended. In the future, I'll probably stay with out-of-network DME providers since my insurance allows me to file out-of-network claims and it's generally cheaper, even at a 20% cpay, then what I would have to pay out of my pocket to a local DME for the same item. Getting the fitting template for the mirage FF mask would be cheaper(maybe even free) from a sleepmall vendor and it isn't that hard to use(usually). My case was a little tough--I'm borderline between a small(which is a little too small) and a medium(which is a little too big). One advantage I had with my local DME fitting me was that they had two sizes already there that I could try out at their office. That made checking the fit of the small and medium mask to my face contour much easier.
