The trick is to configure the nasal pillows so they fit against the tips of your nostrils all the way around. No need to cram them up against your nose, really, although getting them aligned can mean a bit of fussing around.
You will need to fiddle around with the nasal shell. Loosen the thumbscrew and turn the shell inwards until the pillows are nearly flat against the tips of your nostrils. The shell will turn in quite far -- all you have to so is push the little hose down a bit to give it some slack. It helps to look in the mirror and not try to do it in the dark while you are sleepy.
In addition, the position of the pads and the length of the front channel can make a difference. I start by hooking the pillows under my nose and then adjust the rest of the gear so it fits my head. I have the slider that is fastened to the nasal shell pulled out most of the way. I find this makes the front channel springier so it pushes less on my teeth and upper lip. The back pad hooks sort of under the occipital "bump" at the back of my head, and the front (top) pad rests against my upper forehead. (Of course, this is MY head, and YMMV).
In any case, once you get this set up right, there is very little adjustment in subsequent nights.
Oh, yeah, I have my hose hooked up over my head by dangling it from a clamp I put on the bed's headboard. Then I loop a short bungee cord (6 inches) over the clamp and stick the hose through it. This keeps the hose out of my way during the night and allows me to turn over easily. If you don't do this, every time you turn over the hose drags the pillows around on your head and leaks begin.
I can easily go the entire night without a single leak, and I toss and turn with no problem. It's all in the adjustment and getting used to dealing with CPAP. I'm no genius, and if I can do it, so can you (and anyone).
