Obviously a trach would be the better choice between the two. I would guess that while he agrees that a trach might help. He probably is thinking that it might cause more problems if she is not able to care for the trach properly herself. She would need some understanding of what the reasoning behind the trach is, how to care for it, know what to look for so she doesn't get in trouble. Even the simpliest thing as not covering the trach while sleeping. While that sounds easy it's not always. All your head has to do is drop down covering the trach while you are sleeping and the trach doesn't work. I have had my trach for 5 years and it still happens to me occasionally.

There is something called a "transtracheal insufflation (TTI)". It has been a while since I have heard much about it but it seemed to show promise when in clinical trials. I can not post the abstract here nor the contact name and how to reach one of the authors.
