Yes it would be sutured.
At this point it would seem that you need to find a surgeon that has experience with trachs for OSA patients. Look at all the options--pro's and con's. Do some research on tracheostomy on the net. You should be able to find more about the surgery itself, compliations and care there.
While the surgeon will probably be an ENT or Thoratic surgeon, you might want to check out a critical care Pulmonologist. He/she might have experience with seeing improvement with a trach in a person with dementia.
