I was originally Dx'd with an AHI of 52 (mostly hypopneas) and given a plain vanilla CPAP set at 6. I also got careful instructions from the RT on how to set the pressure with a suggestion to try 6 to 8. After achieving some initial successes I found in April / May that I was right back to the exhaustion, fog and headaches. I pushed the pressure up to 9 (10 for one night), but didn't help much. Also, I was having severe GI difficulties and bloating. So I asked for an APAP trial and was given a Virtuoso. The result of the trial (I didn't see the results) was to suggest my settings should be 9. I went several more months at 9 not doing too well and often waking up with air coming out both ends. My decision to buy an APAP was as much to do with lack of progress as bloating, but I find it has helped. I have it set at 7 to 12 and find there are some nights where it never goes above 7.5. Most nights it stays below 9, but sometimes I need the full 12. (Those nights I get bloated, but it isn't often). The nights at 7 I get GREAT sleep. If only you could predict it! With a standard CPAP, based on the output of the AALT I would be set at 9 or 10 - the max pressure required 90% to 95% of the time. With the AALT, I spend most of the time below 8.
