I agree with your post, The Promise of Sleep by Dr. Dement is full of wonderful knowledge on Sleep disorders.
It can be purchased in Sandman's bookstore at the Sleepmall, in fact if you go to the top of Apnea forum home page you'll find a link to the Sleepmall there.
