My chest hurt for the first night or two and then disappeared. I think it might be bacuase you are exhailing against pressure. I get some noise when exhailing, but not excessive and really not much more than when I was using the Adam circuit. Noise is a relative thing so it may be you just need to get adjusted to the normal noises the CPAP makes.
