My phrase that I used when I was so tired as to be nearly comatose was, "I'm falling off my feet tired." Different from mere "sleepiness;" much more profound state of exhaustion for me. Maybe terminology is a large part of the problem. I never told the doctors I was sleepy, always said I was "tired, just so tired all the time."
