Shelle - I started out with the large size and it was huge - it was larger than the large size hole on the template. I called customer support at inomed and they sent me the medium size(just the middle section not a whole new nasal aire. Well, the medium is so much smaller than the large size. It does fit in my my nose, but like you I've had problems with it slipping out at times. I really wish that they had a size in between the medium and large. The large does not fit in my nares at all and the med. is a bit too small. I would call them and ask for the large though, they are really nice about wanting to help and they don't charge for sending another size - so it's worth a try.
