sleep tech needs advice for patient

Posted by nelsleep on July 18, 2002 at 16:28:34:

Hi, I have an elderly lady who has been on CPAP for some months now. She has Moderate OSA, completely normalised on CPAP and also has a host of other conditions such as Diabetes and Heart Disease. Initially she wore CPAP unfailingly for weeks and weeks but said she never slept a wink because of it. Then we found the Breeze sleepgear for her and she slept and immediately percieved the benefit of CPAP. However she has had problems with abraded nostrils with the Breeze and more recently, problems with hayfever like symptoms, nasal discharge, spotting and now last night-a nose bleed for half an hour.
I have had on order for a dreamseal attachment (for weeks) for the soreness in the nostrils. The supplier here is hopeless and I am beginning to think I need to ring his boss to get the thing sent. She is also getting some relief from a cream that has been prescribed.
So... if that works for the sore nostrils by perhaps alternating the dreamseal attachment with the pillows... what then can I do for the sinus/hayfever/discharge/blood spotting/nose bleeds/and general aching sore nasal passages?
That is what is getting her really down.
What I have tried for her so far is humidification (and turning it up) and also asking her to gently warm her room at night as it is winter here in the southern hemisphere.
I have also sent her back to the specialist to see if she has a sinus infection, he just prescribed the cream for the nostrils and smiled a lot.
She tried an Oracle for about 30seconds and declared she could never wear one...ever!
She has also had her bloods done, as my feeling as to the nose bleeds may be due to general unwellness and it being exacerbated by CPAP.
Any ideas...please! Her pressure is 13cmH2O.
