I have an AALT and I HAVE modified the hypopnea definition to try and subjectively improve my therapy. I wouldn't advise this for everyone, but for me after 10 months on "vanilla" CPAP with improvement but lots of distance to go, I wanted to see if I could get either better sleep OR better mornings with an APAP.

By making the definition slightly more "aggressive", I have almost NO apneas but still a lot of hypopneas. Since I react to (at least some) hypopneas, this is not totally what I want, but so far it seems to be the best compromise. It reduced (but didn't eliminate) the mornings I wake up with my stomach bloated so I'm still at a compromise that is not really where I want to be. (BTW, I modify the def slightly and wait a full month before changing it again as my pressure needs vary from nights at my titrated pressure of 7 to nights where I average 11.5 - most nights I average less than 8).

If you set the hypopnea def too aggressive, you get too much response and it defeats the purpose.
