I'm not an engineer, and am not totally sure, but the whole thing is based on moving air. If you're inhaling, you are reducing the pressure necessitating more air movement to bring the pressure back up. When you exhale (breathing against the pressure) you slightly raise the pressure reducing the necessary air flow.
On BIPAP you have two pressure levels, the base or exhale pressure, and the highter inhale pressure. The machine by default is at the lower pressure. When the machine senses the inhale effort, it increases the air movement and you have a higher pressure.
It actually is pretty simple. More air movement, more pressure, less air movement, less pressure. and the good part is, it is all pretty easy to measure.
