13y.o. boy with hypopnea, Insufficient REM sleep, and enuresis

My 13 year old had a polysomnogram last October; at the time, he was unable to fall asleep until many hours had passed, but greatly increased physical activity and, on occasion, 3mg of melatonin have solved that problem. His sleep onset has been delayed since age 6, and that's when he stopped remembering any dream activity. He also stopped bedwetting for 3 weeks at age 9, but resumed, and only an oral dose of .10mg of DDAVP controls this, but has begun to fail recently.

Polysomnogram revealed "delayed sleep, decreased stage REM sleep and decreased total sleep time of 329 minutes, with no significant leg movements during the study." Also, "severe sleep disordered breathing characterized completely by episodes of hypopnea." Baseline oxygen saturation remained close to 90% (he worked his abs like crazy to breathe). During REM sleep, his breathing was noted to be "severe disordered breathing." Conclusion was that his study was "consistent with the dx of severe obstructive sleep apnea/hypopnea.."

He has been found to have no upper airway obstruction, did not notice significant improvement with a mouth splint (didn't use it often or for long, either.. he's forgetful).

He is really socially limited by the enuresis, and I am concerned as well about global effects of this disorder going untreated. Attempts to get actual tx recommendations from the center where my son was tested have failed to yield return calls to me or my son's pediatrician. I notice that the hospital we took him tois not on the list associated with this forum.

We live on Long Island, in NY... how do I proceed, and what are the usual effective treatments for remedying his REM sleep disorder, which, I suspect, would remedy his enuresis... he sleeps so deeply, that alarms have never helped. The point was made to me, repeatedly that he DOES NOT have actual obstructive sleep apnea, just a fragmented pattern just like he does... :-/

Should he see a neurologist, urologist, endocrinolgist?

I'm stumped and he's frustrated.. missing school trips, camps, etc...
