3 yr old keeps waking up

Our 3yr old son wakes up at least once every night, usually 3-4 hours after he goes to sleep, and doesn't fall back into a deep sleep- sometimes he'll keep waking up every 15 minutes for the rest of the night. He hasn't slept all the way through the night since he was an infant, but for the past 6 months he's been increasingly clingy and won't go to sleep unless someone is sitting right next to him. We tried co-sleeping for the past 2 years but he is very restless and hard to sleep next to. For now he starts the night in his bed, then comes into our room to sleep on a mattress on the floor, but he still wakes up crying. He does mouth breathe, but his dr. didn't seem concerned about that- I'm wondering whether this is some sort of attachment thing, since he NEVER wants to be alone... any suggestions?
