6 year old will not go to sleep

My daughter is so sweet and great during the day. She is well behaved overall but when it comes to bed time she freaks out and will not go to sleep on her own. She never really has.. She throws a fit and starts crying and making up reasons for not being able to go to sleep, she is so over-tired. And so are we.

Here's the thing. We have to lay down with her every night, after reading a book in bed with her and winding down the light goes off and she falls asleep but will not let us leave the room. She wakes up when we get out of her bed and starts crying and flipping out! If we do make it out of the room she wakes up around midnight and comes in to our bed and refuses to go back to hers. I know its our fault she relies on us to get to sleep.
I just have no clue what to do about it.
