The plaque could warn of dire consequences of not using the machine, and of the exemption from hand-luggage limits (and cite the exect esction of the law). It can also warn that a prescription is required.
Such a plaque would inform hospital staff and airport/airline workers who are not knowledgable on the subject. Some hospital orderlies, nurses, and even doctors are not knowledgable about OSA. The stern warning on the plaque would help.
