People who had mild OSA just lived with it. People who had severe OSA were problaby labeled as lazy, and had a miserable existance.
Some people may have found limited relief in proping up their head, sewing tennis balls into their pajamas, etc.
Until recently, tonsilectomies were performed routinely, so the incidence and severity of OSA was somewhat less than it is now.

In prehistoric times there was Fred Flintstone (yabba-dabba-doo!). His CPAP was an elephant named Thelma. Sorry, couldn't resist that one.
