How about an auto-titrating BiPAP? A machine that accurately titrates inhalation pressures, but delivers a set and programmable exhalation pressure that is much lower. Or, a machine that accurately auto-titrates a separate inhale and separate exhale pressure based on sophisticated biofeedback algorithms. Now there's an engineering challenge! Some patients experience nPAP prohibitive central apneas long before adequate pressure is reached to address their obstructive apneas. I'd bet an **accurate** auto-titrating BiPAP machine would help many patients who fall into this category.
