Unfortunately, the father's improvement with clonazepam does not prove that even the father has restless legs syndrome. (In addition, per the AASM Practice Parameters, clonazepam is a more effective drug for PLMS than for RLS.)
We have a young man with a hereditary polyneuropathy. I am concerned that the nocturnal discomfort may be either from the restless legs syndrome or the polyneuropathy itself. (The nocturnal discomfort may be a part of the hereditary polyneuropathy as is the peroneal muscular atrophy.)
I would consider a therapy that would treat both RLS and a polyneuropathy.
Unfortunately, I primarily treat adult patients. In this situation, for one of my patients, I would probably choice a medication such as gabapentin. I don't know if there are concerns with this medications in prepubesant children. I do recall a study using the medication for seizure control in children as young a two years of age.
