I've seen this in adrenal failure and also in ammonia buildup in liver failure. Hypothyroidism can do it. Metabolic and endocrine problems need to be evaluated. I had a patient with ascaris infection that had this---also had a high eosinophil count. She really needs a good medical workup. I think the hypersomnia is an effect of some medical problem. Drug screen also needs to be done. JH
