I have personally used one for about a year, and it seems to work wonders for my mild apnea (also have RLS). Recently, a family pet destroyed my mouthpiece. In the process of researching how to find a replacement, I came accross the following URL:

http://bisleep.medsch.ucla.edu/barsh/SDDS.html

If you send a self-addressed envelope to the following address, they will attempt to give you a referral to a dentist in your area that is familiar with construction of the mouthpiece.

Sleep Disorders Dental Society
11676 Perry Highway, Suite 1204
Wexford, PA 15090-8755

Feel free to e-mail me if you have any questions.
