Problem relaxing and falling asleep

When I go to bed or try to take a nap, (and I
am tired), I begin to relax and get drowsy but
before I enter the relaxing doze phase, I get
a mild startle or anxiety feeling that stops my
relaxation and ability to fall asleep. The sensation is akin to when something disturbs
you during your dozing or you think of something
important while falling asleep and you then become alert.

I cannot take naps anymore and my Dr. has
prescribed medication to assist me in going to
sleep. I still do not however experience the
pleasant doze phase prior to going to sleep; I
simply just go from being awake to being asleep
with no dozing.
The anxiety/sensation I feel seems to come
from my abdomen at times.

Is this a known sleeping disorder?
