Posted by John Keith Pruett on January 28, 1998 at 15:08:02:I am John Pruett, a high school student that is conducting a survey dealing with the adverse affects of sleep loss. My mission is to determine a direct link, if any, between a lack of sleep and short term memory loss. To do this, I am trying to reach those people that suffer from a sleeping disorder or simply are not able to get the amount of sleep they need. If you fit into one of these categories I encourage you to fill out the survey below. I know your time might be limited but I would appreciate if you could help me with my research. Your can E-mail me this survey to enven@hti.net.
1. What is your name?
2. How old are you?
3. What is the average amount of sleep you receive per day?
4. About how many days in a row have you followed this pattern?
5. Do you currently have a sleep disorder?
6. Have you had a sleep disorder in the past?
7. What is the name of the sleep disorder? (If not-applicable, then skip)
8. Do you have a chronic problem with forgetfulness?
9. Do you find you have trouble remembering events from the previous day?
10. Do you often forget what you are about to do?
11. Do you frequently forget plans for the next day?Thank you for your time and cooperation.
