Conditioned insomnia

I've suffered with conditioned insomnia for five years, have visited two sleep centers (one for an overnight study which found nothing), eight different professionals (psychologists, psychiatrists, counselors, hypnotists, etc.), and have tried every anti-depressant, anti-anxiety medication known to man. Results? Not! The most sleep I ever get is 5 hours, and I'm a walking zombie all day. I've driven through red lights, stop signs, hit the gas when I should have hit the brakes, etc. I've screwed up royally at work, and my income as a result has dropped precipitously. I need help. I mean, I really, really need help. I'm certain what I have is conditioned insomnia, for a number of reasons, but I don't want to bore you with all the details and background here. Please, can anyone walk me through the steps of getting out of this hellish sleeplessness? I am desperate.
I will do anything! Thanks.
