Have you considered investigating a dental appliance? I'm not sure what you mean by PPO, but if it's a type of insurance maybe you can check it out to see if it will cover one. I can sympathize with your situation because, being a Canadian, I can only have a certain number of sleep studies per year and I've exhausted them all and still have unsatisfactory results. I still have excessive daytime sleepiness and often fall asleep beyond my control at the most inconvenient times. My life is being ruined by sleep. I can't commit myself to anything because I never know if I can get things done. I've even fallen asleep with the stove on and set off the smoke detector. I'm just in the process of getting a dental appliance so I don't know how effective it is, but my Doctor says I may even be able to go without the CPAP. The only reason I've waited this long to get it is cost. Hope this suggestion helps and also knowing you're not alone. E-mail me if you want to talk more.
