After several attempts, I was able to convince my insurance company (Aetna) to pay for my dental appliance. My appliance however, only cost $500 from a dentist affiliated with the Stanford Medical Center. I have used the appliance for over a year now, and have not had any problems with my jaw alignment. There are a couple of issues that you should be aware of though. First, my tongue tends to dry up during the night because the appliance blocks the normal flow of saliva to the tongue. This has led to an extremely annoying "raw tongue" feeling, much like you get after drinking scalding hot coffee. Second, if you have any dental work that changes the shape of your teeth, like caps, you will have to have a new appliance built.
All in all, the appliance has worked very well for me. I still experience the ocassional episode, but at a drastically lower level of severity.
My advice would be to keep hammering your insurance company until they relent. Most of the time you're dealing with some pencil-pusher behind a desk who just doesn't realize what they can and can not do. Try to escalate your request up to a RN. That's what worked for me.
